“The Sixty Plus Program is a program of education, support and services for older adults and their families,” says Nancy Morrison, manager of Sixty Plus Older Adult Services at Piedmont Atlanta Hospital. “We treat the family as a system. Today our services are more important than ever, not only because of the growing number of older adults, but their growing needs,” she says. “Some of the key programs at Sixty Plus are focused on not only the older adult and the family, but also supporting our physicians and our hospital staff.”
The Sixty Plus Program was created 25 years ago by Piedmont to help older adults with the self-management of chronic diseases, educate them about becoming better healthcare consumers and teach them ways to prevent further illnesses or complications.
The Sixty Plus Program services include:
- Advance Directives
- Care Transitions
- Classes & Events
- Caregiver Information & Support Groups
- Clinical Social Work
- Fall Prevention
- Insurance Guidance
- Aging Information Helpline
Community outreach is an important part of the Sixty Plus initiative. Morrison’s team visits senior housing facilities, assisted living centers and church groups to discuss the common issues older adults face.
Thanks to the generous support of donors and the center’s endowment, these resources are free for older adults and their families. Morrison encourages older adults and their families to take advantage of Sixty Plus services.
“This is a tremendous opportunity for them to get some terrific information on aging. All of our staff members are experts in geriatrics, aging and caregiving issues,” she says. Click here for more information about Piedmont Sixty Plus Older Adult Services.
